I saw one with a guy working from a box lifted on the forks of a fork lift truck....







... which in turn was being lifted by a 2nd fork lift truck!

The fork truck looks like work in UK but the rest seem to be in lands far off. As for the fork truck picture they all need sacking including the management. Without the picture you would have difficulty getting people to believe such a thing was possible in a modern factory environment.

My brother uses cherry pickers from time to time and does not use a harness. I said I thought it was HSE ruling that they must be used and any way common sense. You only need to slip once.
