possibly a stupid question, but I'll ask it anyway, and there is reason behind both of these questions as I've DIY tiled a few times, and different methods heed different results
spread adhesive over area wanting to cover (section at a time) or spread the adhesive onto the back of each tile and go that route
also plastic spacers:
directly into the corners as +'s or 2 or 3 down the sides laid as x's and lifted out when adhesive is dry?

adhesive to floor and depending on tiles, back butter these also.
Spacers I put in and take out again. Where you put them depends on the tile. Some of the krap Turkish ones are nominal sizes and hence need to be lined by eye and so may not have spacers to all 4 sides.
One of our tilers who mainly does commercial work and has over 20 years experience doesn't use spacers, full stop on floor or wall tiles. never seen him use a spacer on any of our work. If he wants to leave a 3mm space it is done by eye and amazingly enough if I go round later with 3mm spacers to test his work, it is spot on! He just puts up an area, adjusts, does next area, adjusts the first, 3rd area, adjusts the 2nd area etc etc. Extremely fast way of working, but then again he is a pro and does it day in day out.
