Can anyone tell me the name and type of tree this is? The last 3 years it seems to have disease about this time. I have noticed the same trees in others gardens and they also look diseased?

That's going to depend on the disease.
Some are airborne, some are in the soil already.
Maintaining a good healthy plant is about the best protection, but there are so many possible diseases you probably need identify what it is first.
